Program Details
Community health workers conduct outreach to implement programs in the community that promote, maintain, and improve individual and community health. While serving as a bridge between individuals, the health care system, and social service organizations they provide information on available resources, provide social support and informal counseling, conduct health education seminars, advocate for individuals and community health needs, and/or provide services such as health screenings or contact tracing. This apprenticeship program will provide both classroom and clinical education to build the competencies and skills needed to fulfill this role with in the Alameda County Health System.
